Hello, There are errors in my and my late husband Dale Birley’s Form SSA-1042S-Social Security Benefit Statement. I did pay back $822.00 to your office ($548.00 on November 18, 2019 and $274.00 on January 3, 2020) Dale’s should be $274.00 x 8 months, not 9 months since he passed away on September 28, 2019 and I did pay back $274.00 for September. Please fix it and please send me two new Forms SSA-1042S. Please let me know if this has been taken care of. I am due for income tax by April 30. Thank you very much, Leona Birley, 1619 N. Violet Crescent, Regina, Sask. S4X 4L7
